RAMQ Reimbursement and Certificate of Attendance

As of January 1st, 2018, the RAMQ no longer accepts reimbursement claims in paper format. This must now be done online.

You need to do the following:

1. Register on the RAMQ online system by creating an account at:  https://www4.prod.ramq.gouv.qc.ca/AGS/YR/YRM_GestAuthn/YRM8_CreerCpte_iut/IntrfCreerCpte.aspx
Please have your reference number on-hand.  This is the number that the RAMQ sent to you by mail when you registered with the College of Physicians or other professional Order.
However, if you don't have one or can't find it, you can call the RAMQ at 514-873-3480. Press 1 for "Professionnels de la santé" and enter your professional number. Then press 1 for "Services en ligne". A representative will provide you with a new reference number.

2. Once you have created an account online, you will have access to the RAMQ online services:

The Reimbursement form #3814 can be found under "Facturation - Formulaires" :

When completing your reimbursement claim online, you are not required to attach your certificate of attendance.  However, you should keep it on file for 5 years in case the RAMQ asks you for documentation to support your claim.   To download your certificate of attendance, see below.
